This policy is in relation to Bridge Social Work Services.
Independent Social Work
Collecting Information
As part of providing a social work service, we need to collect information from you and with your consent, from other people and organisations.
This data will include your name, contact details (telephone, email and address), age and date of birth, characteristics and health information.
Sharing Information
A separate consent form will be provided prior to ongoing work which will highlight the relevant external agencies and request your consent for Bridge to contact them.
There are specific situations in which Bridge are duty bound to inform relevant authorities, specifically, if there were safeguarding concerns including risk to self or risk to others. Bridge would inform you of this.
If a referral is made to Bridge Social Work on your behalf, personal information may be given to us by a third party such as a family member, professional or local authority. The information will be processed in the same way.
Purpose of Information
Your information is used for the purposes of providing social work services, including needs assessments, care reviews, mental capacity assessments, case management and support with advance decisions; your personal information is not shared with third parties that are not relevant to providing the above services.
Bridge works as an independent social work service, however, some of the work is within statutory legislation and the information is used for the purposes of providing assessment and report writing.
Storage of Information
All information gathered is stored, transferred and disposed of securely.
· All personal information is stored in a lockable filing cabinet.
· Password protected phone and laptop.
· Password protected documents when sending sensitive information.
· Shredding of personal identifiable information via DIN Level 4 appliance.
· Bridge is registered with the Information Commissioners Office (ICO).
Requesting Removal of Information
You have a right to request deletion/removal of your personal information.
If you would like Bridge to delete your information, please send a request to victoria.tomkins@bridgesocialwork.co.uk – you will receive confirmation that this has been completed.
Subject Access Requests
If you would like to request a copy of the information that Bridge holds about you, it is your right to submit a Subject Access Request (SAR).
Complaints
If you have any concerns regarding how Bridge uses your personal information, you can make a complaint to victoria.tomkins@bridgesocialwork.co.uk
