Costs

The needs assessment is appropriate for clients that are not yet receiving any support and/or are exploring a change due to increased needs or change in circumstances. A report with conclusions and recommendations will be provided following information gathering, consultation and visit/s (as required). 

The care review is appropriate for clients that are already receiving care provision. The review focuses on holistic needs, taking into account client goals, assessed needs, choice and risks. A report with conclusions and recommendations will be provided following information gathering, consultation and a visit. 

Estimated costings can be provided which will be dependent on client needs, goals and social work plan.

Support to document 'Living Will' decisions, complete mental capacity assessment, collaborate and document with health professionals. 

Gathering information pertinent to the decision that needs to be made, ascertaining the salient information, consideration and facilitation of practicable steps to support, visit to complete mental capacity assessment and produce report. 

Statutory assessments within Mental Capacity Act (2005) - gathering information, completion of Form 3, mental capacity assessment, client visit, consultation with interested parties and report writing. 

Non clinical work - charge per hour + 0.45pence per mile
